Reducing risk of diabetes and cardiovascular disease with the Life! program

North Richmond Community Health is currently offering the Life! program, a five session lifestyle group program for people at risk of developing diabetes or cardiovascular disease.

Run by our expert Diabetes Educators, the program can provide you with the information, the motivation and the support to improve your health and maintain an active lifestyle.

The program has helped more than 40,000 Victorians since it began in 2008.

To be eligible for the program, you must fit one of the following categories:

  • 18 years or older who scored >12 on an AUSDRISK tool and have a BMI >25
  • 18 years or older and previously diagnosed with
    •  cardiovascular disease
    • gestational diabetes
    • moderate or severe Chronic Kidney Disease
    • familial hypercholesterolemia
    • serum total cholesterol >7.5mmol/L
    • systolic blood pressure (BP) of ≥ 180mmHg or diastolic BP of ≥ 110mmHg
    • impaired glucose tolerance (by FPG, OGTT or HbA1c)
    • polycystic ovary syndrome
  • 45 years or older, or 35 years or older if Aboriginal or Torres Strait Islander, and have Absolute Risk score of >10%

Patients need to complete a one hour individual assessment with the Life! Facilitator at North Richmond Community Health before starting the group program.

Wednesday 5 July 2017
9.30am to 11.00am
